Nurse Coaching using Habilitation Therapy
Habilitation Therapy is a specialized program developed by leaders in the field of Alzheimer’s and dementia which promotes positive emotions, maximize strengths and minimize limitations for patients and their families. Our nursing Habilitation Therapy coaches have specialized training to employ successful habilitation strategies, improving the quality of life for those living with Alzheimer’s and other dementias. Our coaches can conduct home habilitation assessments, develop individualized plans of care and provide ongoing family support and education.
Our coaching goals include:
- Identifying, understanding, and managing common behaviors
- Helping patients to create and sustain positive relationships
- Maximizing potential for the highest quality of life
- Simplifying daily tasks to encourage patient independence
Supportive Home Care Aide Services
In addition to being certified home health aides, our Supportive Home Care Aides have also received 15 hours of Habilitation Therapy training, so that assistance, whether it be personal care, socialization, meal preparation or housekeeping, can be provided using consistent, compassionate interventions. Their services are available from one to 24 hours/day. All of our Supportive Home Care Aides are supervised by Nursing Habilitation Coaches.
